I understand and accept that participating in Farmer Strong Strength and Fitness, programs and classes involves risks, and I am fully responsible for any injuries or even death that may occur as a result of my participation. I certify that I have no medical conditions that would increase the risk of illness or injury during a fitness program designed by Farmer Strong. With full knowledge of the risks involved, I agree to assume all risks associated with my participation.

By signing this document, I acknowledge that I have voluntarily chosen to engage in a physically demanding exercise program. I am aware of the challenging nature of the program and understand that it may result in unusual physiological effects such as abnormal blood pressure, rhabdomyolysis, fainting, heart attack, or even death. I accept all responsibility for my own health and well-being and release Farmer Strong, its owners, employees, and authorized agents including independent contractors, from any liability or claims arising from my participation. I am aware that I can ask questions and seek recommendations about exercise procedures.

I understand that participating in the activities offered by Farmer Strong involves some level of risk. Therefore, I am willing to accept financial responsibility for any injury I may cause to myself or any other participant due to my own negligence. If Farmer Strong or any authorized representative needs to incur attorney's fees and costs to enforce this agreement, I agree to reimburse them for those expenses.

Moreover, I agree to hold Farmer Strong, their principals, agents, employees, and volunteers harmless and indemnify them from any liability arising from injury or death to any person or damage to property resulting from my negligent or intentional actions or omissions during my participation in the activities offered by Farmer Strong.

I understand that Farmer Strong has unique equipment that differs from what is typically found in a standard gym. I am fully responsible for using this equipment and acknowledge the risks associated with doing so.
